Elucidating the Overlapping Immunometabolic Etiologies of Polyendocrine Metabolic Ovarian Syndrome and Periodontal Disease

Polyendocrine Metabolic Ovarian Syndrome (PMOS) and periodontal disease are among the most prevalent chronic inflammatory conditions worldwide, yet their clinical intersection remains underrecognized. PMOS is increasingly understood as a systemic inflammatory disorder, while periodontal disease is a leading cause of tooth loss. Clinical, biomarker, and epidemiological studies consistently show that women with PMOS demonstrate greater periodontal susceptibility than healthy controls, including elevated probing depths, clinical attachment loss, and gingival inflammation, even when oral hygiene is statistically controlled. Meta-analytic evidence supports a bidirectional association, with PMOS raising the odds of periodontitis by 28 percent and periodontitis raising the odds of PMOS by 46 percent. These findings support integrating routine periodontal assessment into PMOS management and call for interdisciplinary collaboration among dentistry, gynecology, and endocrinology. This literature review synthesizes current evidence on the relationship between these two conditions, focusing on shared immunometabolic mechanisms and the modifying roles of age, metabolic severity, and oral hygiene.
